PROGRAMME REVIEW

The MD programme at PUGScM has been developed to meet both Malaysian needs and international expectations. Various teaching-learning methods are utilised in achieving the learning outcomes of the programme, which include lectures, tutorials, small group discussion, team-based learning, research projects, clinical sessions with standardised/simulated patients, clinical sessions with real patients in hospital settings. Teaching-learning in the first two years of the four year MD programme is targeted at students being provided with a strong footing in the basic medical sciences while being provided with early clinical exposure. The final two years is carried out by a series of clinical clerkships in the various medical and surgical disciplines. Intersession courses are conducted throughout the four years of medical school. Our coursework utilises an online Moodle platform, which provides the student with our exclusive curriculum and direct access to our esteemed panel of lecturers, while delivering numerous exam and quiz questions, as well as offering each student a uniquely tailored step-by-step plan and exclusive test-taking strategies for their success. The unique combination of our exclusive content, as well as student freedom, flexibility, full-time access, and support from our panel of expert lecturers is what sets the PUGScM MD programme apart from others. This gives our students a massive advantage on the road to success on their exam and during the match, if they wish to be matched for residency. Our track record for success is evident not only in the number of student success stories we have accumulated over the past three years, but also in having an over 95% student success rate in the 1st graduated cohort and over 80% success rate in the second graduate cohort, for USMLE, for both Step 1 and Step 2 CK and Step 2 CS. Our 3rd cohort, graduated in 2017. We take each student and evaluate them thoroughly from their first week of their MD programme, ensuring that their areas of concern are addressed immediately, thus allowing us to guide our students both as a whole and on an individual basis. Our programme covers all aspects of the ‘testable’ material as set by the USMLE organization and teaches students what they need to know in order to master the exact skills and strategies required to pass the exam. By purposefully keeping our class sizes small, we provide each student with complete immersion into the different clinical scenarios we present for maximum skill building and success on the actual Step 2 CS exam.

The vast majority of both junior doctors and trainee specialists receive their training in MOH facilities, with no differentiation between those in local or foreign medical programmes. — IMU

There has been recent focus on the parallel pathway in the training of specialists in various medical specialities.

The misinformation and disinformation in some media statements have contributed to confusion, uneasiness, anxiety, and even consternation.

This column seeks to address some of the issues raised.

Registration process

There are about 17,000 doctors in the Malaysian Medical Council’s (MMC) National Specialist Register (NSR).

Just under 8,000 (47%) have Masters qualifications from local universities.

The majority (about 9,000, or 53%) of the other specialists are Members or Fellows of the Royal Colleges of the United Kingdom, Ireland and Australia, with a smaller number having American Board of Medical Specialties certification and other speciality qualifications.

Let us take a look at the specialist registration process in three countries:

The General Medical Council (GMC) of the United Kingdom established its Specialist Register in 1997.

The GMC approved 65 structured speciality and 31 sub-speciality training programmes in UK National Health Service (NHS) and British university hospitals.

In order to complete speciality training – the duration of which ranges from five to seven years, depending on speciality – one has to pass annual reviews and examinations of the respective Royal Colleges.

Upon successful completion, the Certificate of Completion of Training (CCT) is awarded by the GMC.

The Royal Colleges are professional bodies that set the standards, supervise training and conduct examinations for the various specialities.

They have rules and regulations for their examinations, and curricula, which is approved by the GMC.

There are several pathways for GMC specialist registration, i.e.:

  • Combined programme
  • Portfolio pathway – specialist registration
  • Portfolio pathway in a non-CCT speciality
  • Portfolio pathway through academic or research medicine
  • Relevant European qualification – specialist
  • Reassessments under the general system for the recognition of training – specialist, and
  • Qualifying Swiss applicants – specialist.

Speciality training in Singapore has been done in collaboration with the United States (US) Accreditation Council of Graduate Medical Education International since the establishment of Residency Programmes in 2010.

These structured programmes are of three to six years’ duration, depending on speciality.

At the end of the programme, there are assessments before the certification of competency by the Joint Committee on Specialist Training (JCST) is awarded.

There are several pathways for registration with the Specialists Accreditation Board (SAB) of the Singapore Medical Council, depending on the below categories:

  • Specialists trained and certified by the JCST in Singapore
  • Specialists trained and certified in the UK, US, Australia, New Zealand, Canada and Hong Kong
  • Specialists trained and certified in approved centres in European Countries (listed in the European Directive 93/16/EEC), and
  • Specialists with other qualifications recognised by the SAB.

Work-based speciality training is provided in Health Ministry (MOH) facilities for all junior doctors.

About 80-90% of trainees in the local universities’ Masters programmes, which are four-year structured programmes, are trained in MOH facilities.

Their training is no different from that of other doctors, some of whom are in the parallel pathway established in 2014 to prepare them to take Royal College examinations.

Until July 2022, there were two pathways for applications to the NSR:

  • Specialists with qualifications in the NSR list, and
  • Specialists with qualifications not in the NSR list who are considered on an individual basis.

The MMC updated its NSR Registration Procedures and Guidelines , on Nov 21, 2023, in which the pathway for doctors whose qualifications were not in the NSR list were removed.

It appears that the MMC has failed in its duty to inform the profession of changes to its guidelines.

Any change(s) have to be prospective and not retrospective.

Furthermore, it is unfair for doctors to be penalised for any changes not of their making.

What is recognised

A recognised qualification is one that is accepted by a medical regulator as evidence of a doctor’s knowledge, skills and experience to apply for registration to practise.

The MMC has two such lists: primary and specialist medical qualifications.

The former is gazetted and the latter is on the MMC’s website .

Media reports have reflected a misperception of what a recognised qualification is.

This writer’s experience is illustrative of a recognised qualification.

This writer applied for, and was granted, full registration by the GMC, which, at that time, recognised the MBBS (Malaya) degree.

Subsequent to the GMC’s derecognition of Universiti Malaya in the early 1980s, all doctors with the MBBS (Malaya) degree have to pass the Professional and Linguistics Assessment Board (PLAB) examination before they can apply for full registration with the GMC.

An applicant to the Fellow of the Royal College of Surgeons (FRCS) Edinburgh in cardiothoracic surgery was recently informed by the Specialist Applications Advisor of the GMC’s Registration and Revalidation Directorate that he can apply for GMC specialist registration through the portfolio pathway.

This means that this qualification is recognised by the GMC, contrary to claims by certain quarters.

The Medical Act 1971 (amended 2012), which came into effect on July 1, 2017, is the only Act that provides for the establishment of the NSR.

This means that the MMC is the sole authority in the recognition of qualifications for the NSR.

The MMC is not subservient to the Malaysian Qualifications Agency (MQA).

This is clearly stated in the MQA Act, section 3 of which states:

“(1) The provisions of this Act shall be in addition to, and not in derogation of, the provisions of any other written law relating to accreditation of programmes or qualifications.

“(2) Except for professional programmes or professional qualifications , in the event of any conflict or inconsistency between the provisions of this Act and those of any other written law pertaining to accreditation of programmes or qualifications, the provisions of this Act shall prevail and the conflicting or inconsistent provisions of such other written law shall, to the extent of the conflict or inconsistency, be deemed to be superceded.” (italics added)

Chapter 2 (Sections 50-55) of the MQA Act provides that the MMC is the decision-maker in approving or refusing accreditation, imposing conditions, and revocation of accreditation, for medical programmes.

That MMC is the authority in accreditation was reiterated in the case of Asia Pacific Higher Learning Sdn Bhd vs MMC Anor (2019).

Branch campuses of foreign institutions physically present in Malaysia, i.e. Monash University and Newcastle University, have been accredited by MMC.

Both institutions have also been accredited by the Australian Medical Council and GMC respectively.

MQA accreditation does not mean automatic MMC recognition, e.g. UCSI University’s Master of Science (Healthy Ageing, Medical Aesthetics & Regenerative Medicine) programme is accredited by MQA, but is not recognised in the NSR list.

The qualifications in the parallel pathway are still found in the NSR list.

This means they are recognised.

The MQA itself has acknowledged that the authority on the parallel pathway issue lies with MMC, and not itself.

The term “recognised training institution” in section 14B of the Medical Act has been used in arguments against recognition of the parallel pathway.

The contention is that MOH facilities are healthcare providers, and not recognised training institutions.

This argument ignores the fact that MOH provides work-based training for all junior doctors, including 80-90% of Masters trainees.

There have been no reports of double standards in training for Masters trainees, those on the parallel pathway, and those on no pathway.

If MOH facilities are not recognised training institutions, does this mean that the qualifications of the Masters trainees in MOH facilities should not be recognised also?

It is needed

Patients, colleagues and friends have asked this writer why the issue of the parallel pathway has only surfaced 10 years after it started (in 2014) and seven years after the NSR was established (in 2017).

What interests drove the opposition to the parallel pathway?

Were they personal, institutional, professional or racial?

That it is not professional is reflected in the tremendous support for the parallel pathway by the professional organisations.

Malaysia is short of specialists, more so in some disciplines like cardiothoracic surgery.

The Masters programmes, which have been around since the 1980s, have not produced sufficient specialists for the country’s needs and are unlikely to do so in the foreseeable future.

The parallel pathway matters.

Dr Milton Lum was an elected member of the MMC from August 1995 to July 2022, as well as a past president of the Federation of Private Medical Practitioners Associations and the Malaysian Medical Association. A/PROF DR ZUKARNAIN ZAKARIA

Associate Professor and Programme Director BSc (Resource Economics) UPM, MEc (UKM), PhD (Southampton, UK)

“The global healthcare sector is among the most dynamic and rapidly growing industries in the world economy.   The health industry has become a powerful engine of economic growth. Healthcare industry can be a robust economic engine and one that indirectly creates significant social impact. The Malaysian health sector growth prospect looks extremely attractive. The Sector is expected to grow to MYR127 billion (US$30 billion) by 2027, fueled by increasing demand for healthcare services from an ageing population, rising affluence, and increasing life expectancy. Spurred by demographic shifts such as extended longevity and a rise in lifestyle, diseases such as hypertension and cardiovascular ailments, cancer and diabetes, national healthcare costs are also increasing dramatically.   The IMU doctoral programme in Healthcare Management is designed to develop scholars that can apply theoretical frameworks from the social sciences (such as economics, organisational theory and management, sociology) to address pressing health policy and management research questions. The programme is purposefully interdisciplinary and seeks to prepare students for research careers in academics as well as in public and private sector agencies and organisations. ”
