• Incomplete submission forms will not be considered.<br />
• Complete the form enclosed in BLOCK LETTERS only.<br />
• Use Black/Blue Ball pen only to fill up the form.<br />
• Fees for thesis Evaluation is Rs. 3000/-.<br />
(Fee to be paid at any Indian Bank Branch across India through pay-in-slip/challan. NBE<br />
copy of challan in original has to be affixed with this form)<br />
• In case of re-evaluation, a fee of Rs. 1500/- will be paid by candidates<br />
along with re-submission of their thesis.<br />

• DNB candidates are required to submit their thesis before the cut off date<br />
which shall be 30th June of same year for candidates appearing for their<br />
scheduled December final theory examination. Similarly candidates who<br />
shall be appearing in their scheduled June DNB final examination shall be<br />
required to submit their thesis by 31st of preceding December.<br />
• Any change in your correspondence address after submission of thesis<br />
should be intimated to the <strong>Board</strong> at email [email protected],<br />
[email protected] and [email protected] so as to affect timely<br />
dispatch of your thesis acceptance letter and other communications.<br />
• For inquiries pertaining to thesis, you may write to<br />
[email protected]. No inquiries regarding thesis acceptance status<br />
prior to 4 months after thesis submission will be entertained.<br />
• Mention your Name, Subject, Registration No. and Date of submission of<br />
thesis in any correspondence pertaining to thesis with the <strong>Board</strong>.<br />
