“Savita Malhotra did her graduate medical degree, and then postgraduation (MD) degree in Psychiatry and PhD from the Post Graduate Medical Institute of Medical Education and Research (PGIMER) in Chandigarh India.  She has been a British Council Fellow and WHO Fellow for training in child psychiatry in UK and USA. She is a Fellow of Academy of Medical Sciences and an Honorary Fellow of American College of Psychiatrists, and of the World Association for Social Psychiatry.

Received Yves Pelicier Prize 2022 of the World Association for Social Psychiatry, the highest award and the most coveted award in psychiatry in the world, for stellar contribution to social psychiatry. 

She retired as Dean, and Prof & Head Department of Psychiatry at PGIMER Chandigarh. She has 45 years of experience in clinical practice, education and research in psychiatry. She pioneered the development of child psychiatry in India for >30 yrs; and conducted pathbreaking research on development of a model telepsychiatry application for delivering mental healthcare in remote areas; conducted seminal research in various fields such as acute transient psychoses, long term course and outcome of schizophrenia, child and adolescent psychiatry among others. Has been a teacher to over 200 MD students. Known in academia in India and abroad. Published >350 research papers, books and book chapters.”

ICMR Scheme for MD/MS-PhD Programme

ICMR being an apex research agency for formulation, coordination and promotion of Biomedical Research in the country is also engaged in promoting critical mass of human resources as well as fostering research temperament amongst medical graduates. Amongst its several Human Resource Development programmes the Council has reinstituted Talent search scheme of providing financial support to selected 'Medical Graduates/ Postgraduates'. The scheme has the approval of its Governing Body (GB) held on 2nd August 2000 and 11th July 2003.

The programme is designed to identify young medical graduates with brilliant academic records for pursuing postgraduate qualifications & to motivate them to opt for career in its research cadre. The selection of the candidates will be made on the basis of a competitive National level examination consisting of a written test conducted at 3 or 4 Centres in the country. The candidate passing all MBBS examination in the first attempt with 60% or more aggregate marks will be eligible for the examination.

Under this programme, the selected medical graduates would be provided financial assistance for 4-5 years and a total of 25 fellowships will be available per year. (Programme is ongoing at three Centres viz., King George's Medical University (KGMU), Lucknow; National Institute of Mental Health and Neurosciences (NIMHANS), Bengaluru and SRIHER (Sri Ramachandra Institute of Higher Education and Research), Chennai .

The highlights of scheme are as follows:

  • The Council will provide financial support equivalent to Junior Resident's salary+ allowances on a running pay scale for 3 years followed by Senior Resident's salary + allowances on a running pay scale for 2 years to selected candidates.
  • Number of Awards: Limited to 25 per year.
  • Eligibility: Medical graduates who are selected and admitted through national level test to the MD/MS-PhD courses in above stated (three) MCI recognized Institute/College and who wish to avail of the ICMR award for pursuing their research project.
  • Upper age limit: Max.45 years.

1. Procedure for Selection

1.1 The scheme would be widely publicized though advertisement in leading news papers, circulars to the medical institutions and ICMR website once a year. This will be the responsibility of the host organization. 1.2 A nominee from Director General, ICMR should be placed in the interview. 1.3 It will be done through written competition followed by interview. 1.4 Selection would be made on the basis of merit. In the event of leaving this scheme without completing the programme the whole amount will be remitted to the Council with suitable justifications. 1.5 The candidate and the guide will indicate their choice of the research topic and will give a brief description of the objectives, methodology, design of the study, expected outcome, relevant and recent literature review, ethical considerations and facilities available at the Host institute for conducting the research study as per the proposed protocol and an undertaking by the Head of the institute that the candidate will be permitted to undertake the proposed research activity (within two years).

2. Financial Support

2.1. The ICMR would provide a total support for five years duration to the selected candidates 'on the condition' that the candidate would submit following documents to the ICMR:   • An undertaking to the effect that the funds received from the Council will be used strictly for the purpose for which it has been released, and in the event of leaving this scheme without completing the programme the whole amount will be remitted to the ICMR with suitable justifications.   • A surety bond will be executed by the candidates entering into the programme on Rs. 100/- non-judicial stamp paper.   • The original certificates will be retained and it will not be returned to the candidates before completion of the course unless he /she deposits a sum of Rs. 50,000/-. 2.2 A copy of the award letter issued by the Vice chancellor / Director of institute. 2.3 A copy of the thesis protocol submitted to the University/Institute as a part of the research programme. 2.4 An electronic copy of the same. 2.5 A short summary of the thesis (2-5 pages) prepared by him, outlining the objectives, methodology, findings and discussion. 2.6 Part-I thesis to be submitted for fulfillment of requirement of MD/MS at two and half years (2½), from the joining date and examination at three years for MD degree after acceptance of thesis by two examiners (for MD+Ph.D.) 2.7 Part-II thesis (final) to be submitted for fulfillment of Ph.D. degree at four and half years (4½) from the joining date and thesis defense at five years after acceptance of thesis by three examiners (for MD+Ph.D.) 2.8 If registered for Ph.D. after completing MD (condition 2.6 & 2.7 is not applicable). However in this case, the total duration of MD-Ph.D. fellowship will be three years only, instead of five years. Final Ph.D. thesis to be submitted after completion of three years. 2.9 If registered for Ph.D. after MBBS (condition 2.6 & 2.7 is applicable) 2.10 The selected fellows will be given Junior Resident's grade salary+ allowances on a running pay scale for three years followed by Senior Resident's grade salary + allowances on a running pay scale for two years & contingency grant of Rs.1,00,000/- per annum (for point no. 2.8 contingency will be for three years only). The financial assistance would be provided in yearly installments. Statement of expenditure will be required for releasing next installment. 2.11 Same rules and regulations will be followed as existent for Junior Residents/Senior Residents in Central Government Institutions/Autonomous Institutes. 2.12 The grant shall be released in the name of the Head of the Institute directly with a copy to the Guide and the candidate. 2.13 The amount will be used by the candidate for pursuance of research and may use the funds for purchase of reagents, preparation of thesis, secretarial assistance or any other activity connected with the research project following fair and reasonable procedure. 2.14 Utilization Certificate has to be submitted to ICMR regarding the manner in which the funds were utilized duly signed by the Accounts Officer and Head of the Institute/college.

3. Monitoring

3.1 The fellowship can be terminated at any time on a month's notice, if the progress of work is not satisfactory or on receipt of an adverse report from the Guide. The fellowship can be terminated forthwith, if the particulars given in the application form for fellowship are found to be incorrect or false. 3.2 The local institutional committee (approved by ICMR) and Guide should review his/her case annually whether extension for another year should be given. The Director-General, ICMR, will make the final decision on grant of extension.

4. Submission/Publication of Thesis/Research Papers

4.1 The preparation and submission of thesis will be the responsibility of the student and his/her guide. Candidate is required to publish minimum five papers/publications in International indexed Journal. The candidate will acknowledge the fellowship provided by ICMR for any publications emerging out of the thesis work. 4.2 The candidate should submit documents listed in item No.2.1 within given period to the institution/organization failing which, the award will be treated cancelled and all financial assistance provided will be reimbursed to the DG, ICMR.

  • Selection of candidates at the sole discretion of Council
  • The scheme can be withdrawn at any time.
  • This scheme is not linked with any other research capacity building schemes under which the students are in receipt of stipend etc.

The rise of super (?sub)-specialties courses in psychiatry: Is India ready for it !

The field of psychiatry is still growing;[ 1 ] however, the phenomenal growth occurred in many areas in the field of Indian psychiatry in last decade. One of them is the increase in postgraduate seats (Diploma in Psychological Medicine (DPM)/Doctor of Medicine (MD)/Diplomate of National Board (DNB) of psychiatry to address the shortage of psychiatrists in our country. In curious latest development two medical institutes in our country started two super-specialties psychiatry courses (after branching out from department of psychiatry) in the academic year of 2011-2012 such as Doctor of Medicine (DM) (Geriatric Mental Health) at ‘King George Medical University’, Lucknow ( http://www.kgmu.org/dept_geriatric.php ) and DM (Child and Adolescent Psychiatry) at ‘National Institute of Mental Health and Neurosciences’ (NIMHANS), Bangalore ( http://www.nimhans.kar.nic.in/cap/default.htm ) for candidates of MD/DNB psychiatry. Other awaiting super-specialty psychiatry course is DM (Addiction Medicine) at ‘National Drug Dependence Treatment Centre’ of ‘All India Institute of Medical Sciences’, New Delhi and ‘Centre for Addiction Medicine’, NIMHANS, Bangalore.

  • In an outlook it is a welcome step, however, authors consider this rise of super-specialties in still growing field of Indian psychiatry is a debatable issue. When our country is already facing 77% average national deficit of psychiatrists,[ 2 ] will not this step lead further shortage of general psychiatrists in India? India is currently producing 357 MD (Psychiatry) and 129 Diploma in Psychological Medicine (DPM) per year in various medical college/institutes of our country.[ 3 ] Along with these reasons and the migration of qualified psychiatrists to developed countries, will this step be a pragmatic one for MD psychiatry candidates to enter DM in these super-specialties?
  • Then, what is the need of having super-specialty psychiatric courses? Is there a demand for clinical service, if so, having separate clinic/unit in the psychiatry department (one of the requirement of Medical Council of India (MCI) to start MD psychiatry course) shall be sufficient. Is there a demand in research point of view, if so, developing a separate 3 years course is not justified? If the development for teaching purpose due to lack of expertise, having separate clinic/unit within department of psychiatry would suffice. Considering all above points, Indian academic psychiatry is not ready for premature branching out as 3 years DM course. It may be pragmatic to wait till required number of psychiatrists of India is available for clinical service.
  • Another important aspect of this development is the employability after their training. Authors tend to believe as not so lucrative for these super-specialties as there are already inadequate practicing general psychiatrists and inadequate teaching psychiatric faculties in medical colleges.
  • In view of public mental health, when India is struggling to provide primary care psychiatry and facing difficulty in the implementation of ‘National Mental Health Program’, can we afford to have these super-specialties in psychiatry which further reduces the human resources?

PROGRESS IN INDIAN MEDICAL EDUCATION WITH RESPECT TO SUPER-SPECIALTIES

Authors feel that the developing new courses with new name are becoming fashionable in the field of Indian education. Medical education of our country is not the exception for this and is already developed new courses in last few decades such as DNB, DM, Magister Chirurgiae (MCh), etc., Irony in this development is having same name (i.e., ‘Doctor of Medicine’) for two abbreviated courses MD and DM with different hierarchy.[ 4 ] The MD is postgraduate course called as broader specialty for MBBS candidates and DM is postdoctoral course called as super-specialty for MD and even for MBBS candidates. In the document of MCI 2015 vision, both these courses are recommended and do not have expansion for DM abbreviation.[ 5 ]

Even prefix ‘super’ in the nomenclature of so called ‘super-specialty medical degree’ is a debatable issue. The use of prefix ‘super’ commonly observed in various commercial industries such as ‘super-market’ in retail industry, ‘super-model’ in fashion industry, ‘super-star’ in film industry, ‘super-specialty hospital’ in health care industry, ‘super-built’ area in real estate industry, etc., No industry has legal sanctity for prefix ‘super’. Unfortunately, prefix ‘super’ is enjoying legal sanctity in medical education by both MCI and National Board of Examinations.

Probably, these courses are heading for another peak of commercialization.

SUGGESTIONS

  • Authors argue for change of nomenclature in prefix ‘super’ to ‘sub’ and prefers to call them as ‘sub-specialties’ rather than ‘super-specialties’ psychiatry as these are branching out from psychiatry with narrow area of interest.
  • Authors suggest for replacing this 3 years DM course to 1 year post-doctoral fellowship (PDF) in all desirable sub-specialization within department of psychiatry. Meanwhile, MD psychiatrists with PDF in any sub-specialty may be accommodated within department of psychiatry and encourage to pursue research career in these sub-specializations within the department of psychiatry. This reduces the chances of branching out from parent department and ensure the availability of expertize for clinical services, teaching, as well as for research purpose. Over a period of time, faculties of department of psychiatry will have MD psychiatry with one PDF in one of these sub-specializations, so that department will have psychiatrists having different sub-specialization. If research is main issue for so called super-specialties, then Doctorate degree as Doctor of Philosophy (PhD) in these sub-specialties may be encouraged. If clinical service is reason, one year PDF may be preferable, so that 3 MD psychiatrists will have expertise training in the same 3 years. PDF or PhD shall continue in main psychiatry department rather than having separate department for these sub-specialties.

The rise of super-specialties in psychiatry looks to be boon for Indian psychiatry. It could be a bane for field of psychiatry as it is leading to premature branching out and potential danger for commercialization. Author strongly suggest for change of nomenclature from super-specialty to sub-specialty. India is not ready for 3 year super-specialization academic degree out of parent department of psychiatry, instead, authors suggest for 1 year PDF in all desirable sub-specialties within department of psychiatry.

M.Phil in Psychiatric Social Work:

The department conducts 2 year M.Phil. (Psychiatric Social Work) Course. Not more than 18 to 21 candidates per year may be admitted for the course. Of these, 3 seats are for foreign nationals / Deputed candidates from State Government / Government of India.

Ph.D in Psychiatric Social Work:

Ph.D. guidance facilities are also available in the department. A candidate who has obtained MSW and/or M.Phil in Psychiatric Social Work degree from any recognized University or any other University recognized as equivalent thereto by NIMHANS securing not less than 55% marks can register for Ph.D. examination after one year from the date of provisional registration. The candidate would be trained in research methodology, theoretical and practical aspects of the curriculum on Ph.D for a duration of 3 – 5 years.

One year fellowship in psychosocial support in disaster management:

Started in the year 2012-13, it has 8 seats, with four seats for deputed candidates focuses on building the capacity of the trainees in integrating psychosocial support services in disaster management programs.

Training for Social Workers / Fellows from different countries:

The department has also been involved in the training of social workers from several counties like Nepal, Bangladesh, Srilanka, USA, UK, Canada, Tanzania, Yemen and Burma including WHO Fellows.

Block Placement Training for Social Work Students:

The department also trains the Post Graduate students undergoing M.A. / MSW programmes from other Universities in Karnataka and other States for period ranging from 2 weeks to 6 months. Not less than 400-600 students get exposure to Psychiatric Social Work activities every year.

Marit UyHam

Originally from Atlanta, Georgia, Marit UyHam is a rising sophomore at Dartmouth College. She plans to study biology and hopes to attend medical school.  At Dartmouth, Marit works in a biological anthropology lab which analyzes microfossils with a focus on prehistoric China.  Outside of class, Marit is involved in multiple dance programs, and she plays violin with the Dartmouth Chamber Orchestra.

Amal Sharif

With over six years of experience in healthcare,  Amal Sharif has dedicated her career to improving patient outcomes through innovative approaches. Having worked at Highland Hospital, a Level 1 trauma center in the East Bay, Amal has firsthand experience in high-pressure medical environments and understands the critical importance of effective communication and empathy in patient care. Amal holds a Mathematics, Psychology, and Economics degree from Laney College. Amal enjoys exploring her creativity through various artistic pursuits, such as pastel, and drawing.

He has a background in theatre and comedy improv and has integrated his interests in medicine and the arts through hosting local events for medical students to share their experiences on stage via Stethoscope Stage and HuMed Short Story Night in partnership with TCU Burnett School of Medicine. Additionally, he is collaborating with TCU in establishing the Narrative Medicine Consortium of Texas to unite Texas medical schools in increasing Narrative Medicine education. His work has been featured on The Nocturnists Podcast, MedMic.com, and Crohn's & Colitis Young Adult Network. Peter plans to pursue Psychiatry with interests in Child & Adolescent Psychiatry, Eating Disorders, and GI-adjacent Psychiatry. 

Keren Shafer

Keren Shafer is a rising MS1 at the John Sealy School of Medicine -UTMB Health-. She is pursuing a medical and master’s in public health degree as a stepping stone to becoming a pediatrician or OBGYN. She graduated with a Distinguished History degree with a double minor in Biology and Chemistry. Her interest in Historical writing includes women’s, Chinese, and medical history. Holding a vested interest in the medical humanities, her archival research has explored how intimate narratives of embodiment, contextualized through health and illness, are both particularized and shared as part of the human condition - the very stories inscribed in the histories of our humanity. While continuing to grow as a writer, she plans on applying to medical school, aiming to discover and put into practice what it means to care for another in all aspects of being.

Nada Kaissieh holds a Masters of Bioethics from Johns Hopkins University and is currently advancing her medical education at the University of Louisville School of Medicine, working towards her MD. Soo Yeon (Jean) Chun is a rising junior at Stanford University planning to major in Symbolic Systems on the Neuroscience track. Since middle school, she has been fascinated by the creative, emotional, and linguistic capabilities of the mind. An aspiring psychiatrist and writer, she is deeply interested in the power of creative writing—particularly poetry —to guide and heal. Maria Luiza Fernandes is a sophomore undergraduate student from Brazil. She is graduating in Pharmacy and plans to become a neuroscientist. Her research interests cover a range of disciplines under the umbrella of the pharmaceutical profession and cognitive science. As an Immerse Education fellow, over the past year she has worked on a research project on Alzheimer's disease, including the applications of gene editing in the treatment of pathologies associated with the nervous system. She is currently involved in a learning community on psychopathologies and an executive member of FLOTA, a project aimed at developing young female leaders around the Americas.

Robinrenee Hamre

Robinrenee Hamre is a sophomore undergraduate student at UCLA, majoring in Biology. She is a Native American student, originally from Anaheim, California. Robinrenee is passionate about studying Neonatology and pursuing a career in the medical field, in hopes to become a NICU Doctor.  Some of her hobbies are writing, running, and reading poetry.

Jess	Skyleson

Jess Skyleson (they/them) is a former aerospace engineer and Ayurvedic practitioner who began writing poetry after being diagnosed with stage IV cancer at age 39. Currently in remission, they’re now pursuing an MFA in Digital + Media at Rhode Island School of Design, with particular interests in narrative medicine, computational poetry, and sonic art. Their poetry has appeared in journals and anthologies throughout the US and UK, and they have been awarded the 2022 Hippocrates Poetry and Medicine Prize, an Honorable Mention in the Tor House Poetry Prize, and were a finalist for the Yemassee Poetry Prize and Kalanithi Writing Award.  They are presently exploring the integration of the body, poetry, and sound, and one of their sound poetry projects was recently selected for exhibition in the New Media category at Brown University’s Ivy Film Festival. Jess facilitates creative writing and art workshops for patients, medical providers, and caregivers, and they are hoping to develop collaborative pathways across art mediums and personal/professional experiences of medicine.

Emily Koseck is a medical student at Queen’s University in Canada. She is currently working at Toronto Metropolitan University on the development of a new medical school with an innovative approach to education that will meet the current pain points in the healthcare system.
