In brief: what is speech therapy.

Created: August 12, 2020 ; Next update: 2024.

Speech therapy can help people who have difficulty speaking to communicate better and to break down the barriers that result from speech impediments. The goals of speech therapy include improving pronunciation, strengthening the muscles used in speech, and learning to speak correctly.

Speech therapy can be used for a lot of different speech problems and disorders, from smaller problems like a hoarse voice up to partial loss of speech due to brain damage. Depending on the type of disorder, other medical or psychological treatments may be used as well.

  • What kinds of disorders can speech therapy treat?

Speech therapy can be used to treat language disorders, speech disorders and swallowing problems.

Language disorders

A childhood language disorder can affect the child’s ability to learn to speak, to name objects and build complete sentences. Although the causes of these disorders are often not clear, the main known risk factors include hearing problems , general developmental problems and disorders affecting the development of the brain.

Language disorders in adults are almost always the result of brain injury or disease. People who have had a stroke , for example, often have trouble forming sentences or remembering words. That type of disorder is called aphasia.

Speech disorders

People with speech disorders have difficulty producing the sounds of speech, saying words clearly or talking fluently.

Children often have trouble with pronunciation, and may have a lisp or swap certain sounds for others. Speech disorders may be the result of developmental disorders, but psychological factors might also play a role. Adults with neurological diseases sometimes have speech disorders too, often making it hard to understand them.

Another group of speech disorders, known as fluency disorders, involve problems with the flow or evenness of speech. People with this sort of disorder may stutter or “clutter,” for example. When people stutter, there are often silent pauses in their speech, or they repeat or lengthen certain sounds or syllables. Cluttering is abnormally fast speech that makes the pronunciation imprecise or leaves out sounds or parts of words.

Voice disorders (dysphonia)

A voice disorder is a persistent change in someone’s voice. They might sound hoarse, strained, raspy or nearly silent. Often the voice is somewhat weak – in other words, it cracks easily or the person is not able to speak loudly. Voice disorders may arise from speaking too much or too loudly, from using the wrong breathing technique, or from problems with the voice box (larynx) like vocal nodules . Psychological causes like depression or a reaction to a distressing event can change a person’s voice too.

Trouble swallowing

In people with swallowing problems, the movements of the muscles involved in swallowing are affected. This leads to problems transporting food through the mouth and throat. The cause is often a disease or disorder of the nervous system, such as Parkinson’s disease , multiple sclerosis, dementia , an infection like Lyme disease or tetanus, or a head injury. If food gets into the lungs because of a swallowing disorder, it can lead to life-threatening complications.

What treatments are used in speech therapy?

There are various speech therapy techniques for each of the areas described above – the ones that are considered depend on the particular disorder. A long series of treatment sessions is typically needed, with each lasting 30 to 60 minutes. They may take place in a group or one-on-one.

The treatment approaches used in speech therapy include:

  • Perception exercises, for example to differentiate between individual sounds and syllables
  • Exercises to produce certain sounds and improve the fluency of speech
  • Exercises to improve breathing, swallowing and the voice
  • Help with communication using things like sign language, communication boards and computer-assisted speech
  • Advice for people who need speech therapy, their parents and other loved ones
  • Support in implementing these measures in everyday life

For the treatment to help over the long term, it’s often important to also regularly practice the techniques at home.

  • Where is speech therapy offered?

Speech therapy is offered at the following facilities:

  • Speech therapy practices
  • Rehabilitative care centers
  • Special needs schools
  • Children's day care facilities specializing in speech therapy

Besides speech therapists, there are a number of other specialists who also use similar methods. These include breathing, speech and voice coaches.

  • Do statutory health insurers cover the costs of speech therapy?

Note: The procedures and requirements for applying for and receiving speech therapy may vary according to your country. This information describes the current situation in Germany.

To have outpatient treatment at a speech therapy practice, you need a prescription from a doctor. An initial prescription will generally include up to 10 treatments, each typically lasting 30 to 60 minutes. Appointments are usually offered one to three times per week.

For the medical conditions listed above, prescribed speech therapy is often covered by statutory health insurers (apart from a fixed amount that you have to pay yourself, known as a copayment). The copayment does not have to be paid when getting a treatment that has been prescribed for children. If speech therapy is given as a part of rehabilitative care, an accident insurer or pension fund will cover the costs.

The copayment that you have to pay for yourself is 10 euros per prescription plus 10% of the treatment costs. If each treatment costs 55 euros, for example, for ten treatments you would have to pay 65 euros (the basic fee of 10 euros per prescription plus 10 x 5.50 euros).

Some speech therapists may offer certain treatments without a prescription. You then have to pay for all of the costs yourself. In Germany, these are known as individual health care services (individuelle Gesundheitsleistungen, or IGeL for short).

The German Federal Association of Speech Therapists (DBL) has a search function for speech therapists on their website (in German).

Finding a speech therapist in China

I am having problems pronouncing some of the Chinese sounds, e.g. the "r" initial in 日, and even some initials that I am pronouncing legibly are still somewhat non-native sounding. My Chinese is also very "throaty" (for lack of a better description.) Since I have been in China for a good few months now, I am starting to suspect that I will not be able to resolve this on my own.

Now, don't get me wrong, I am not having problems being understood, it's just that I would greatly prefer to sound more like native Chinese. My Chinese teachers are therefore less-than-helpful: their standard for what constitutes acceptable pronunciation is too low for me.

So I want to hire a Chinese speech therapist to improve my pronunciation. Preferably someone who is ridiculously strict, and who can tell me exactly what I'm doing wrong and how to fix it.

Does any forum members have any experiences regarding this? Any ideas on how I might find a person like this in Harbin?

Thanks in advance,

Unless you think there's an actual problem with the way your mouth works, I don't think finding a speech therapist is the answer, you just need a better teacher. Try a few private schools, make it clear that you want a trial lesson focusing on the issue and stick with whichever teacher is best.

If you want to go to a bit more effort, find a trainer for the 普通话水平测试, which Chinese people take to prove their own level of standardness - ie for broadcasting jobs.

Ah, "speech therapist" was probably not what I meant, was thinking of the Swedish word "talpedagog" which seems to have no direct equivalent in English.

Thanks for the 普通话水平测试 suggestion. I'm going to try that route, but they will probably just laugh at me; my Chinese really is nothing to brag about.

I managed to find the following address of a 普通话 training and testing institute in Harbin:

黑龙江普通话培训测试中心,电话:0451-82607760

地址: 南岗区中宣街20-8号

公交线路: 乘7、72、112路到中宣街站下车

Going to work up some courage and head over there maybe tomorrow.

"Preferably someone who is ridiculously strict, and who can tell me exactly what I'm doing wrong and how to fix it."

I had a wonderful and extraordinary teacher just like that who'd originally trained as a Chinese actress. So I too would suggest you look for people who teach at acting and broadcast schools and such. This is a field in which age and experience count.

if no problem pronouncing 'zhi' and 'chi', then 'ri' is no problem. just roll tongue back a little when pronouncing 'zhi' or 'chi', and you get the 'ri'.

drag 'zhi~~' then roll tongue back, and you get it.

By the way, in English I'd call this sort of teacher not a "speech therapist" but a "speech coach" or a "voice coach." Or maybe a Henry Higgins.

Roddy is right.

This semester I had three teachers.

One said that my pronunciation was good.

One said that I needed to work on the flow of my sentences, to practice identifying the natural pauses, etc.

One said that about 40% of the sounds that came out of my mouth were wrong for some reason or another, and was very helpful in patiently telling me how I should open my mouth, where my tongue should be, etc., for every sound.

Now that I think about it, I'll need to give that teacher #3 a call.

I still have night terrors about my pronunciation fixing sessions. I think the highest praise I ever got out of the (actually extremely lovely) woman was something along the lines of 'you are starting to approach a standard I might perhaps be able to consider accepting.'

Ideally you want a middle-aged lady who's been doing this for years, is very sparing with praise, brooks no nonsense, and is a great big softie underneath it all.

Meng Lelan

Where do you find her?

Mine did 普通话测试 training at an education college in Dalian, though I found her via a private school there. Anywhere they train elementary school teachers, broadcasters, etc.

If the teachers you already have can do it, great, but so many of them have that 'good enough for a foreigner' attitude. I had one university Chinese teacher tell me once that I wouldn't be able to get my pronunciation spot on due to 退化 of the relevant vocal organs. I tried to explain that it was laziness that was the real issue, but she couldn't understand me.

I tried to explain that it was laziness that was the real issue, but she couldn't understand me.
Why could she not understand?

skylee

Anyway, if your teachers are too accepting, you simply need to look for people who are less accepting. Even a tandem partner who has great pronunciation and is really picky. Some people simply have that "no no no, wrong wrong wrong" attitude and will go on about minute details, and you need to find one of those.

A trained professional is of course preferable, but even a regular student who is annoyingly picky will get you very far.

Glenn

Am I the only one who thought he was making a joke about his pronunciation being so bad she couldn't make out what he was saying? Because that's how I read it, and I laughed for about five minutes straight.

I didn't see anything funny about it at all.

Pancake, did you get anywhere with this?

And while we're waiting for some response from pancake ---- Any Chinese speech therapists looking for, uh, online speech therapy assignments?

I went to the address I posted earlier in this thread. Unfortunately, they were not equipped to help this 外国人 with a less-than-perfect command of Chinese. They are primarily a testing centre, and can only help those who only need to standardise their putonghua a little bit.

Any Chinese speech therapists looking for, uh, online speech therapy assignments?

Is that a double entendre or do I just have a dirty mind?

The people who actually give the tests are likely also trainers, if you can persuade someone to put you in touch. I had a quick look and there's a 黑龙江教育学院 in town as well.

Pledges and promises are coming thick and fast from every party as the general election approaches.

Struggling to keep up with who is saying what?

Here is a summary of where the main parties stand on major issues.

For a more in-depth look at what each party has pledged, scour our  manifesto checker ...

TV presenter Rylan Clark has said he would "love" to become a politician - and replace the party system with a "Power Rangers of government" model.

The TV personality, 35, joined political editor Beth Rigby and former Scottish Conservative leader Baroness Ruth Davidson for this week's Sky News Electoral Dysfunction podcast.

Asked if he would ever consider the career change, he said: "If I wasn't in the job that I was in, I would love nothing more."

Rylan, who won Celebrity Big Brother and also appeared on the X Factor, appeared on the podcast in place of Labour candidate Jess Phillips after tweeting his praise for Rigby on the day Rishi Sunak announced the general election.

Sharing a clip of her and Sky presenter Sophy Ridge outside a rainy Downing Street waiting for Mr Sunak to appear at the lectern, he said: "Obsessed with the Rigby."

Speaking to her and Davidson, he said his "obsession" with politics began with Brexit - "as we've seen so many promises which weren't fulfilled" since then.

He added: "I lie there at night sometimes, and I think about [Volodymyr] Zelenskyy. He hosted one of the same shows I've hosted in Ukraine."

The TV presenter also shared his idea of abandoning political parties altogether.

A short while ago, Rishi Sunak gave a speech BAPS Shri Swaminarayan Mandir, also known as Neasden Temple, which is a traditional Hindu temple in London.

Rishi Sunak, who is of the Hindu faith, told attendees: "Like all of you, I draw inspiration and comfort from my faith.

"I was proud to be sworn in as a member of parliament on the Bhagavad Gita [one of Hinduism's most prominent sacred texts].

"Our faith teaches us to do our duty, and not fret about the outcome as long as one does it faithfully."

"My family also strengthen my commitment to service," the PM said.

He spoke about the philanthropic work that his mother-in-law does in India, and praises his wife as "the greatest support that any husband could ever have" and "someone committed to a life of service".

To the attendees, Mr Sunak said: "I want to start by thanking all of you for your support, your prayers, and your love. You have been with me every step of the way.

"On the hardest days in this job, I have felt your backing, and I know the pride that it is to have a British-Asian prime minister, and I am determined to never let you down."

The PM argued that so many Indians are Conservatives because "our values bind us together", citing education, hard work, and family.

Mr Sunak said: "One of my most memorable moments of the last few years came on Diwali, sixty years after my [grandmother] boarded a plane in East Africa - her first ever, all by herself - to come ahead and make a new life for her family here in this country.

"Sixty years after that moment, her great-granddaughters, my girls, they played in the street outside our home."

They "had fun like so many other families do on Diwali - except, the street that they were playing in was Downing Street".

In reference to the racial slur a Reform UK campaigner had used about him, he said that the "events of the last few days should not make us forget that the United Kingdom is the world's most successful multi-ethnic, multi-faith democracy, and we should all by uplifted by that".

In conclusion, the PM said: "Thank you for all your support. I can only hope that I can make you all proud every day."

Even by Reform's standards, the frequency and ferocity of attacks flying out from the party has stepped up today.

We've had complaints going into Ofcom, the Electoral Commission and Essex Police.

The BBC boycotted, Channel 4 reported, and a former campaigner cut adrift.

This tells you more about Nigel Farage's mindset than his decision to suspend three candidates over online posts.

So, five days from the election, will these seemingly rolling controversies shift many votes?

Much like the row over the Reform leader's comments on Russia and Putin, that probably depends on how fully signed up you are to the party’s agenda.

For the diehards, talk of an establishment stitch-up will find sympathetic and supportive ears.

But wavering Tories dabbling with Reform may be queasier about all this talk of racism and a big media conspiracy.

One final point.

A fortnight ago, Nigel Farage demanded to be treated as one of the big players in this election, citing a poll putting him ahead of the Tories.

But with more coverage comes more scrutiny.

You can construct a fair argument that that's exactly what Reform has been exposed to in the last week.

We've also had the results in from a poll carried out by Opinium for The Observer.

Similarly to the Savanta poll we mentioned in our previous post, it found Labour was still in the lead and the Conservatives remain unchanged.

However, it found support for Labour hadn't dipped and was still the same at 40%.

Reform UK were up one point to 17%, the Greens fell three points to 6% and the Lib Dems and SNP remained unchanged.

Labour has fallen four points to 38% - the party's lowest share since Rishi Sunak became prime minister - in a Savanta poll for the Sunday Telegraph.

But, it still holds a 17 point lead over the Conservatives, who were unchanged on 21%, indicating a landslide majority for Sir Keir Starmer.

"This is our lowest Labour vote share since Rishi Sunak became prime minister, and the Conservatives for their part do appear to have slowed or stopped their downward spiral," said Savanta's political research director Chris Hopkins.

"However, that's where the good news ends for the prime minister, as if this vote share was replicated on polling day, Labour would still likely have a majority of over 200."

Here's how some of the other parties did in the poll:

  • Reform UK unchanged on 14%
  • Liberal Democrats up one point on 11%
  • Green Party up one point on 6%
  • SNP down one point on 2%
  • Others up one on 7%

Mr Hopkins said the increase in vote share for the Greens and independent candidates is "notable".

"Not because it will have an impact on this election in any significant way, but could point towards trouble for the Labour Party in government and over the coming years," he added.

Some 2,092 UK adults were polled from 26-28 June. The comparison is with an earlier poll conducted from 21-24 June.

Thank you for watching tonight's edition of Politics Hub With Ali Fortescue .

We heard from:

  • Mark Spencer , Conservative candidate and farming minister;
  • Sir Anthony Seldon , educator and contemporary historian.

And on the panel were:

  • Max Wilson , former Labour political adviser;
  • Claire Pearsall , former Tory adviser.

Scroll down for all the key moments and highlights - and stick with us here in the digital Politics Hub for the latest political news throughout the evening.

Sir Anthony Seldon is our next guest on Politics Hub With Ali Fortescue .

He has penned a new book entitled '2010-2024: 14 wasted years?', and we ask for his reflections on the Tory years in power.

He says there were some good things, such as in education where standards rose, "aspects of pensions, aspects of science, aspects of the arts, Universal Credit".

But he goes on: "Overall, growth and productivity has been stagnant since the global financial crisis of 2007-8, and if we look at health, if we look at transport, if we look at housing, if we look at the state of defence, if we look at Britain's position in the world - we don't see the different historians and academics who wrote the book... there isn't the kind of significant change in the standard of all those various areas and other that we have tended to see in long periods of Tory government in the past."

He says there have been "some progressive, important, and incremental changes, but overall a disappointing performance, frankly".

"It's hard to think of any period of single-party domination by the Conservative Party that has achieved less than the party's achieved since 2010."

Infighting has been partly to blame, and it is also a party that is "unsure what it believes in" and is "confused", Sir Anthony says.

He also says "the flip flop" of policies and ideological direction has been a problem.

"There were so many opportunities that the Conservative government could have had for consistent and thoughtful policymaking, and it simply hasn't happened."

Despite the "stability" brought by Rishi Sunak and Lord Cameron, Labour is on track for "a very significant victory" on Thursday.

He notes that no party since 1832 have ever won five general elections in a row, and adds: "It is going to be a colossal Labour victory, akin to 1945."

Celebrities endorsing political parties is not a new phenomenon, but Sir Elton John publicly backing Labour today has got people thinking - do they actually help things shift?

Max Wilson, former Labour political adviser, tells the Politics Hub With Ali Fortescue that endorsements from the famous are a "good thing".

But, he says that they are unlikely to "shift the dial" or increase the number of votes.

"It is a nice thing to have, but I don't think it is a prerequisite to winning," he adds.

Former Tory adviser Claire Pearsall feels the idea has "really gone out of fashion".

"It has sort of slid away over the years... I would be more impressed if it was sort of Margaret from Burnley telling us exactly why she's voting for whichever party," she adds.

"We're now in the realms of understanding that real people want to see people like them."
