Total quality management is the continual process of detecting and reducing or eliminating errors in manufacturing, streamlining supply chain management, improving the customer experience, and ensuring that employees are up to speed with training. Total quality management aims to hold all parties involved in the production process accountable for the overall quality of the final product or service.

Total quality management was developed by William Deming, a management consultant whose work had a great impact on Japanese manufacturing. While Total quality management shares much in common with the Six Sigma improvement process, it is not the same as Six Sigma. Total quality management focuses on ensuring that internal guidelines and process standards reduce errors, while Six Sigma looks to reduce defects.

The focus of the process is to improve the quality of an organization’s outputs, including goods and services, through continual improvement of internal practices. The standards set as part of the Total quality management approach can reflect both internal priorities and any industry standards currently in place.

Industry standards can be defined at multiple levels and may include adherence to various laws and regulations governing the operation of the particular business. Industry standards can also include the production of items to an understood norm, even if the norm is not backed by official regulations.

Introduction • Quality management (QM) and quality improvement (QI) are the basic concepts derived from the philosophy of total quality management (TQM). • Now it is preferred to use the term Continuous Quality Improvement (CQI) since TQM can never be achieved. • The method of monitoring of healthcare for CQI is done with Quality Assurance (QA).

Definition • Quality assurance is a judgment concerning the process of care based on the extent to which that care contributes to valued outcomes. - Donabedian 1982

General approach • It involves large governing or official bodies evaluating a person or agencies‘ ability to meet established criteria or standard during a given time.

a) Credentialing • It is the formal recognition of professional or technical competence and attainment of minimum standards by a person and agency. • Credentialing process has 4 functional components • To produce a quality product • To confirm a unique identity • To protect the provider and public • To control the profession

b) Licensure • It is a contract between the profession and the state in which the profession is granted control over entry into an exit from the profession and over quality of professional practice

c) Accreditation • It is a process in which certification of competency, authority, or credibility is presented to an organization with necessary standards. d) Certification Confirmation of certain characteristics of an object, person or organization.

e) Charter • It is a mechanism by which a state government agency under state law grants corporate state to institutions with or without right to award degrees. f) Recognition It is defined as a process whereby one agency accepts the credentialing states of and the credential confined by another g) Academic degree

Elements According to Donabedian; • Structure Element- The physical, financial and organizational resources provided for health care. • Process Element- The activities of a health system or healthcare personnel in the provision of care. • Outcome Element- A change in the patient‘s current or future health that results from nursing interventions.

According to Manwell, Shaw, and Beurri, there are 3A’s and 3E’s; • Access to healthcare • Acceptability • Appropriateness and relevance to need • Effectiveness • Efficiency • Equity

Indicators of quality assurance

Waiting time for different services in the hospital • Medical errors in judgment, diagnosis, laboratory reporting, medical treatment or surgical procedures, etc. • Hospital infections including hospital- acquired infections, cross infections. • Quality of services in key areas like blood bank, laboratories, X- ray department, central sterilization services, pharmacy and nursing.

Quality assurance cycle:

QI process steps include; • Identify needs most important to the consumer of health care services. • Assemble a multidisciplinary team to review the identified consumer needs and services. • Collect data to measure the current status of these services.

Establish measurable outcomes and quality indicators. • Select and implement a plan to meet the outcomes. • Collect data to evaluate the implementation of the plan and achievement of outcomes.

Factors affecting quality assurance in nursing care:- 1. Lack of Resources 2. Personal problems 3. Unreasonable patients and attendants 4. Improper maintenance 5. Absence of well informed populace 6. Absence of accreditation laws

TOOLS OF THE CQI:

Pareto Charts: • The Pareto chart analysis is used when dealing with chronic problems and helps one identify which of the many chronic problems to attack first.

Fishbone Diagram: • One analysis tool is the Cause-and-Effect or Fishbone diagram. • These are also called Ishikawa diagrams because Kaoru Ishikawa developed them in 1943. • They are called fishbone diagrams since they resemble one with the long spine and various connecting branches.

Histogram: • This is a vertical bar chart which depicts the distribution of a data set at a single point in time. • A histogram facilitates the display of a large set of measurements presented in a table

Run chart: • Most basic tool to show how a process performs over time. • Data points are plotted in temporal order on a line graph. • Run charts are most effectively used to assess and achieve process stability by graphically depicting signals of variation.

PONTE VEDRA, Fla., June 3, 2024 /PRNewswire/ --  Cadrenal Therapeutics, Inc. , (Nasdaq: CVKD), a biopharmaceutical company developing tecarfarin, a late-stage novel oral and reversible anticoagulant (blood thinner) designed to prevent heart attacks, strokes, and deaths due to blood clots in patients with rare cardiovascular conditions, today highlighted a groundbreaking presentation at the International Society for Heart & Lung Transplantation (ISHLT) 44th Annual Meeting & Scientific Sessions.

These new findings from secondary analyses of the ARIES-HM3 trial were released in a presentation titled, "Impact of Vitamin K Antagonist (VKA) Therapy On Outcomes In a Randomized Controlled Trial of Aspirin Removal In Left Ventricular Assist Device (LVAD) Patients - A Pre-Specified Analysis From the Aspirin and Hemocompatibility Events With a Left Ventricular Assist Device in Advanced Heart Failure, or the ARIES-HM3, Randomized Clinical Trial."

The ARIES-HM3 trial data demonstrated that lower time in therapeutic range, or TTR, translated directly to excessive bleeding events. The "average" patient in the ARIES-HM3 study had a 30% rate of serious bleeding events even after aspirin was eliminated as part of the antithrombotic regimen, and persistent bleeding was inversely correlated with TTR.

The ARIES-HM3 clinical study was sponsored by Abbott (NYSE: ABT), which evaluated a new clinical approach to patient management that included removal of aspirin as part of the antithrombotic regimen warfarin. The data is currently under review by the FDA. Labeling changes related to the antithrombotic regimen have not been approved by the FDA at this time.

Dr. Mandeep Mehra, who chaired the ARIES-HM3 study, holds the William Harvey Distinguished Chair in Advanced Cardiovascular Medicine and is Executive Director of the Center for Advanced Heart Disease at Brigham and Women's Hospital, commented, "This comprehensive analysis identifies adequacy of VKA use (as measured by TTR) as a significant risk marker for bleeding events and provides new clinical direction for further mitigation of bleeding to enhance hemocompatibility with the HeartMate 3 LVAD." Mehra continued, "Each incremental improvement of 10% above the median of 56% TTR trends in a significant further reduction in bleeding rate. Tecarfarin could potentially be an important therapy for patients with LVADs who all require chronic anti-coagulation since it does not get affected by drug-drug interactions or changes in kidney function like warfarin and deserves further study."

We believe our drug candidate, the next-generation VKA tecarfarin, with its unique retrometabolic design that provides for more stable anticoagulation than warfarin, is the much-needed replacement therapy. We intend to pursue a pivotal trial evaluating tecarfarin effectiveness for LVAD patients," said Quang Pham, Founder, Chairman and Chief Executive Officer of Cadrenal Therapeutics. On April 9, 2024, Cardenal Therapeutics announced that the United States Food and Drug Administration (FDA) had granted tecarfarin Orphan Drug Designation for the prevention of thromboembolism and thrombosis in patients with an implanted mechanical circulatory support device, which includes the left ventricular assist device (LVAD).

The current market-leading direct oral anticoagulants (DOACs), such as Eliquis, are not indicated for patients with LVADs due to a lack of evidence of benefit, while the level of anticoagulation achieved with warfarin was achieved in the target range only 56% of the time in the ARIES-HM3 trial. Patients whose level of anticoagulation was in the therapeutic range > 56% had better outcomes than those with lower levels. This highlights the potential role for investigating new VKA agents in improving clinical outcomes in LVAD patients.

VKA anticoagulation is prescribed for the prevention of LVAD-related clotting. However, the only available VKA is warfarin, which was approved for human use in 1954. Tecarfarin has been shown to improve TTR, particularly in patients taking multiple medications, and be more stable in patients with renal dysfunction which is common in LVAD patients.
