When I was younger, I wanted to be a pediatric oncologist. Although I didn’t end up going to med school, I found ways to study oncology through health psychology and public health, and now through medical and science writing. I remain fascinated by oncology and cancer books in all forms: textbooks, memoirs, novels, graphic novels, narrative nonfiction, and more. Here are some of my must-read cancer books, in no particular order.

The Undying: Pain, Vulnerability, Mortality, Medicine, Art, Time, Dreams, Data, Exhaustion, Cancer, and Care by Anne Boyer

This book…is a powerhouse. I’ve read a lot of cancer memoirs, and I even hesitate to call this a “cancer memoir” because it is so much more than that: it is an indictment of how we see and treat cancer, especially breast cancer. It is a sharp observation of our healthcare system and how it fails when needed most. Boyer, a single mother, was diagnosed with triple-negative breast cancer, a highly aggressive form of the disease. Her writing takes on the pharmaceutical industry, art and illness, capitalism and cancer, and more. I find it hard to describe this book the way I’d like, because it is so layered and so damn smart. I would put it down often as I read, because I had to think through what Boyer wrote.

The First Cell: And the Human Costs of Pursuing Cancer to the Last by Azra Raza

This book is a must-read. It explores the costs of cancer research and treatment—both financial and human, and how despite millions and millions of dollars spent in research and clinical trials, the majority of cancer drugs in the last few decades only minimally extend life, and in a significant percentage, are actually harmful to people. Her compassion for her patients is evident in her writing, and her anecdotes about her work are exquisite and carefully, thoughtfully drawn. She is passionate about her work and the need to reframe cancer research, clinical trials, and patient care, as well as the overhaul that is much needed in oncology.

The Emperor of All Maladies: A Biography of Cancer by Siddhartha Mukherjee

This book is one of my all-time favorites. Mukherjee is a master at writing about complicated things in a way that brings them to life in an interesting way. He can make a single cell the most fascinating thing in the world, and he combines memoir, reportage, and research seamlessly. A Pulitzer Prize winner, this book explores the history of cancer and its treatments, as well as the future of cancer treatment.

Tough: Women Who Survived Cancer edited by Marquina Iliev-Piselli

This is a collection of stories of women with a variety of different cancers. The tone is decidedly positive—yes, there’s honesty in the essays, but it does skew toward the “cancer changed my life and is a gift” kind of story. Some essays are better than others, but overall, this is an interesting book that gives voice to women with a variety of cancers, which is always needed.

Cancer Vixen: A True Story by Marisa Acocella Marchetto

I read this years ago and it has stayed with me ever since. A New Yorker cartoonist, Marchetto depicts her journey of her breast cancer diagnosis and treatment through this graphic novel. Engrossing, well-drawn, and even humorous (she calls her mother her “(s)mother”), I highly recommend this book. Marchetto doesn’t shy away from anything, and this is a refreshing take on the breast cancer memoir.

The Bright Hour: A Memoir of Living and Dying by Nina Riggs

Riggs, a poet/essayist and a descendant of Ralph Waldo Emerson, wrote this quietly beautiful book about her breast cancer, which was published posthumously. At first, it was easily treatable, and a year later, it metastasized. Her observations about living while dying, raising her children while facing her mortality, and how she wants to live in the time she has left are wry and brutally honest. She never becomes sentimental or saccharine, and overall, this is not a depressing read. Riggs actually got in touch with Paul Kalanithi’s widow Lucy, and formed a friendship with her, which eventually led to Riggs’s widow and Lucy falling in love.

The Immortal Life of Henrietta Lacks by Rebecca Skloot

The HeLa cell was the first immortal human cell obtained in a lab. Taken without her knowledge or consent from a poor tobacco farmer named Henrietta Lacks who was being treated for cervical cancer, the cell has become one of the mainstays of scientific research—and yet her family was in poverty. Skloot tells the story of Lacks, as well as race, medicine, and ethics. Read this before seeing the HBO movie. Skloot is a fantastic science writer.

Memoir of a Debulked Woman: Enduring Ovarian Cancer by Susan Gubar

Ovarian cancer is a devastating cancer. It’s often not diagnosed until it’s widespread, because the symptoms are so nonspecific and there’s no good standardized screening tool. We don’t get many stories of women living with or those who have survived ovarian cancer—but this is one of them. Gubar tells her story honestly, describing the radical debulking surgery that was used to help treat her ovarian cancer, and searches for meaning in the only ways she knows how. She examines the way ovarian cancer is treated and how it affects people and their families, and shies away from nothing while telling her story of survivorship.

When Breath Becomes Air by Paul Kalanithi

If you haven’t read this memoir yet, do it soon. Kalanithi was finishing up his neurosurgery training when he was diagnosed with Stage IV lung cancer, and this book is an exploration of what it means to be on both sides of the diagnosis. What happens when a doctor suddenly faces imminent mortality? How do you reframe what is really important in life when you never expected it to end so soon?

Being Mortal: Medicine and What Matters in the End by Atul Gawande

This is not just a book about cancer, but about end-of-life issues and care and the decisions we make in treatment and not treating people and their diseases. He interweaves personal and professional stories with research and science, and brings up the point that when it comes time for death and end-of-life decision making, medical care is often counter-intuitive to what it should be. Treatment and prolonging life above all else, avoiding conversations about the realities of illness and death, and over-treating to the point of impairing quality of life and in some cases, hastening death. As usual, Gawande is spot-on with his brilliant insights and compassionate takes on difficult issues. Highly recommended, along with all of his other books.

Radical: The Science, Culture, and History of Breast Cancer in America  by Kate Pickert

This memoir recently came out in October 2019 and it is fabulous. Pickert, a health journalist, was diagnosed with aggressive breast cancer at the age of 35. She details her treatment and the options she had, examines the way we approach screening, prevention, and treatment in America, including the “awareness” and pinkwashing of breast cancer, and the societal aspects of the disease. What I really loved about this book was her exploration of metastatic breast cancer and how it is often ignored by society, activists, awareness, and the breast cancer community.

The Unwinding of the Miracle: A Memoir of Life, Death, and Everything That Comes After by Julie Yip-Williams

This is a hard book to read, but worth it. Yip-Williams was diagnosed with metastatic colon cancer at the age of 37, with two small children. She started a blog, and this book grew out of that blog. She tells it all: the treatment decisions, what the treatments are like, the emotional rollercoaster of her cancer and her knowledge of her imminent death. She writes about being an immigrant from Vietnam and how her grandmother wanted to euthanize her because of her blindness, and how eventually, when she got to the U.S., surgery gave her some sight back. She writes about wanting her husband and girls to enjoy life without her, her fears for them, her anger, and her love. She knows she is dying and never pretends otherwise, but this book isn’t about death; it’s about life.

The Philadelphia Chromosome: A Genetic Mystery, a Lethal Cancer, and the Improbable Invention of a Life-Saving Treatment by Jessica Wapner

The Philadelphia chromosome was discovered in 1959, and in 1990 it was found to be the cause of chronic myeloid leukemia (CML). Wapner details decades of scientific research and practical applications of scientific findings, blending reportage, research, and narrative to make this a compelling read, even if you’re not a scientist.

The Cancer Chronicles: Unlocking Medicine’s Deepest Mystery by George Johnson

For science writer Johnson, cancer was just another story subject—until the woman he loved was diagnosed with metastatic cancer. Johnson decided to learn everything he could about cancer and oncology, and this book is the result. Written in clear prose, this book explores metastasis, mutations, cells, chromosomes, statistics, laboratory studies, clinical treatment, carcinogens, the history of cancer, the future of cancer treatment, and everything you could possibly think of. He examines cancer from multiple viewpoints and disciplines, and the result is one of the most interesting books I’ve read on cancer.

Swimming in a Sea of Death: A Son’s Memoir by David Rieff

This was written by Susan Sontag’s son, and while not explicitly about cancer itself, he explores the last nine months of his mother’s life, from her diagnosis through her death from cancer. If you’re a fan of Sontag’s writing, this book is a tender character exploration of Sontag from a unique point of view—but at the same time, Rieff manages to avoid sentimentality. He creates a picture of a smart, strong woman trying to beat the odds of her disease and living (and dying) on her own terms.

The Middle Place by Kelly Corrigan

Corrigan was in that “middle place” that many Baby Boomers find themselves in: caring for their children and managing their household, while also starting to take care of their parents in some ways. She was a thriving adult but still saw herself as her father’s daughter. She was diagnosed with breast cancer, and shortly after her diagnosis, her father was also diagnosed with prostate cancer—late-stage cancer. This memoir is a beautiful, funny (yes, funny) story about family relationships and taking care of the ones you love.

The End of the Beginning: Cancer, Immunity, and the Future of a Cure by Michael Kinch

Kinch has experience as both a university cancer researcher and a head researcher at a biotech company, and his focus has been the immune system and its role in oncology. This book explores the interplay between the immune system and cancer and its treatment in clear prose that even a non-scientist can understand. The level of detail Kinch goes into is fascinating, and I’m eager to see what comes next with immunotherapy treatments in oncology.

Everything Happens for a Reason: And Other Lies I’ve Loved by Kate Bowler

At the age of 35, Bowler, a professor at Duke Divinity School, is diagnosed with stage IV colon cancer. Her specialty is the study of the prosperity gospel, which sees every good thing as a blessing from G-d, and any bad thing that happens as divine disappointment or disapproval. Her diagnosis, then, throws her into a kind of crisis—she has assumed she can control her life, and has lived with a kind of rugged individualism that assumes that anything can be overcome or achieved if you just work hard enough, are positive enough, or are deserving enough. This book examines what happens when that just isn’t always true.

The Anatomy of Hope: How People Prevail in the Face of Illness by Jerome Groopman

I cannot say enough about this book. The first time I read it, I carried it around with me in my bag for weeks. Don’t let the title mislead you: this is not about false hope. Groopman is not positing that hope is a cure-all; rather, he looks at how hope factors in to illness in a variety of ways, even if it doesn’t look like you think it does.

The Cancer Journals by Audre Lorde

Lorde, a poet, kept a record of her breast cancer journey, and this book is one of my favorites. A mix of memoir, journal entries, and criticism, this is a necessary record of a feminist and LGBTQ experience through breast cancer. The language is gorgeous (with Lorde, this is a given), and this should be required reading for everyone going through cancer, no matter what the kind.

The study of the biology of tumours has grown to become markedly interdisciplinary, involving chemists, statisticians, epidemiologists, mathematicians, bioinformaticians, and computer scientists alongside medical scientists. Oxford Textbook of Cancer Biology brings together the developments from different branches of research into one volume. Structured in seven sections, the book starts with a review of the development and biology of multicellular organisms, how they maintain a healthy homeostasis in an individual, and a description of the molecular basis of cancer development. The book then illustrates how, once cells become neoplastic, their signalling network is altered and pathological behaviour follows. Changes that cancer cells can induce in nearby normal tissue are explored, and the new relationship established between them and the stroma is explicated. Finally, the authors illustrate the contribution provided by high throughput techniques to map cancer at different levels, from genomic sequencing to cellular metabolic functions, and how information technology with its vast amounts of data are integrated with traditional cell biology to provide a global view of the disease. The book concludes by summarizing what we know to date about cancer, and in what direction our understanding of cancer is moving.

The Cancer Survivor’s Companion: Practical Ways to Cope with Your Feelings After Cancer

By lucy atkins.

The Cancer Survivor’s Companion is a remarkable book that offers a fresh and insightful perspective on navigating the emotional rollercoaster that follows a cancer diagnosis. Lucy Atkins, a highly respected author and journalist, delves deep into the complex world of cancer survivorship, providing practical advice and support for those grappling with their feelings after cancer.

Atkins skillfully weaves together personal stories, expert interviews, and evidence-based research to offer a comprehensive guide that is both informative and inspiring.

One of the book’s unique and unpopular ideas is the importance of embracing vulnerability and allowing oneself to experience a range of emotions. Atkins challenges the notion that cancer survivors should always remain positive, encouraging readers to acknowledge and process their fears, anger, and grief. By doing so, she argues, individuals can cultivate resilience and find a deeper sense of healing.

The Cancer Survivor’s Companion also delves into the concept of post-traumatic growth, a lesser-known phenomenon where individuals not only recover from the trauma of cancer but also experience personal growth and positive life changes. Atkins explores various strategies and coping mechanisms to help readers harness this transformative potential, empowering them to create a meaningful and fulfilling life beyond cancer.

Through its compassionate and empathetic tone, The Cancer Survivor’s Companion is a beacon of hope for those affected by cancer. Atkins provides practical exercises, self-reflection prompts, and resources to help readers navigate their emotional landscape with grace and resilience. A woman follows a large golden pill as it breaks through a wall towards greenery

Cancer kills almost ten million people annually. According to one study, this disease, in all its many forms, will cost the world a whopping 25 trillion international dollars — an artificial currency used to compare economies — over the next 30 years ( S. Chen et al . JAMA Oncol . 9 , 465–472; 2023 ). More than 50 years after the US declaration of a war on cancer , many hoped that the disease would be closer to defeat than these statistics suggest. People diagnosed with cancer today often still contend with the dismal side effects and highly uncertain outcomes associated with the decades-old therapeutic gauntlet of radiation, surgery and chemotherapy. New treatments for cancer are needed to turn the tide decisively — and they are rapidly arriving.

Nature Outlook: Cancer treatment

Laboratories across the world are delivering a steady stream of encouraging — and in some cases astonishing — results. Several of these treatment approaches enlist the immune system in innovative ways. After many years of trying to develop therapeutic cancer vaccines, some researchers are starting to find success with in situ vaccines , which generate an immune response to a person’s tumour inside their body. Others are weaponizing natural killer cells , which attack tumours and produce fewer side effects than T-cell therapies. Other immune components — antibodies — are proving useful as guided missiles that carry conventional chemotherapy drugs directly to cancer cells .

By combining medical tasks, theranostics — a combination of therapy and diagnostics — is scoring impressive results . The idea is to send radioactive particles into the body to identify and locate cancer, and then follow up with a different class of emitters that can deal the cancer cells a death blow. Researchers are also exploring the use of artificial intelligence to help work out which therapy is best for an individual. With new treatments emerging from labs so rapidly, it might be time to rethink the clinical-trial system lest people be deprived of the most effective treatments available.

High fiber diet.

Aelia Akbar ; Aparna P. Shreenath .

Affiliations

Last Update: May 1, 2023 .

  • Definition/Introduction

A high fiber diet refers to a diet that meets or exceeds the Dietary Reference Intake (DRI) for dietary fiber set by the United States Institute of Medicine (IOM).

The current daily DRIs for fiber are as follows [1] [2] :

  • Children ages 1 to 3 years: 14 grams (g)
  • Girls 4 to 8 years: 16.8 g
  • Boys 4 to 8 years: 19.6 g
  • Girls 9 to 13 years: 22.4 g
  • boys 9 to 13 years. 25.2
  • Girls 14 to 18 years: 25.2 g
  • Boys 14 to 18 years: 30.8 g
  • Women aged 19 to 50 years: 25 g
  • Men aged 19 to 50 years: 38 g
  • Women aged 51 and older: 21 g
  • Men aged 51 and older: 30 g
  • Issues of Concern

Most Americans consume only half the levels of recommended fiber per day, which is almost 15 grams per day.

There is not a single universally accepted definition for fiber. In 1953, Hinsley first defined dietary fiber as a “non-digestible constituent that makes up the plant cell wall.” [3]  Since then, the definition of dietary fiber has been promulgated in various ways by different countries and organizations. [4] [5]  Defining fiber is elusive because new compounds are identified that have chemical properties of fiber but lack its physiological properties or vice versa. People eat fiber for its health benefits; therefore, it is preferable to have a definition of fiber based on its physiological properties, but for food industries, scientists and researchers prefer to rely on definitions set by the Association of Official Analytical Chemists (AOAC). [6] [7]  All existing definitions recognize fiber as “carbohydrate or lignin which bypasses digestion in the small intestine and is partially or completely fermented in large intestine or colon.” [8] [5]

Dietary fibers are defined as follows:

Based on availability in nature,

  • Dietary fiber: This consists of non-digestible carbohydrates and lignin that are intrinsic and intact in plants. The term non-digestible refers to the material that is not digested or absorbed in the human small bowel. Non-digestible plant carbohydrates are components of the plant cell walls and intercellular structures that maintain their three-dimensional properties even after mechanical treatment. [9] [10]
  • Added fiber: These are indigestible carbohydrates isolated or extracted from animal or plant sources and have beneficial physiological effects in humans. These include attenuation of blood glucose and cholesterol levels and improved laxation. Synthetically manufactured non-digestible carbohydrates like resistant starch are also included in this category. An essential aspect of defining added fiber relative to its physiological benefits is that extraction or isolation of fiber by chemical means can either enhance or diminish its beneficial effects. Any fiber synthesized in the lab cannot be listed under fiber until it demonstrates the health benefits. [9] [10]

Based on physicochemical properties;

  • Soluble fiber: They are water-soluble and derived from the inner flesh of plants such as pectin, gums, and mucilage. They form a viscous gel and are usually fermented by bacteria in the colon into gases and by-products such as short-chain fatty acids. They alter the blood glucose and cholesterol concentrations. [5]  Sources of soluble fibers are:
  • Legumes, peas, and beans
  • Vegetables such as broccoli and carrots
  • Most root vegetables

2. Insoluble fibers: They derive from the outer skin of plants. They are insoluble in water and usually cannot undergo fermentation by bacteria in the colon. As a result, they form the bulk of the stool and promote laxation. Cellulose, hemicellulose, and lignin are insoluble fibers. [5] Sources of insoluble fiber are:

  • Cellulose:  Corn bran, potatoes, and skin of most fruits from trees such as apples, banana, and avocado, many green vegetables such as zucchini, green beans, celery, and cauliflower, some fruit plants such as tomatoes and kiwi.
  • Hemicellulose: whole grains
  • Lignin: nuts and seeds
  • Clinical Significance

A high-fiber diet has the following health benefits:

  • Bowel movement: Insoluble fiber maintains bowel movements. They absorb water and softens the stool. Soft stool is easier to pass, thus preventing constipation. They also add bulk to the stools hence prevent the formation of loose stools. [11]
  • Improved bowel health: High fiber diet prevents the formation or worsening of hemorrhoids and diverticular disease, which presents as outpouchings in the colon walls.
  • Studies have shown that a high-fiber diet decreases the risk of colorectal cancer. [12]
  • Cholesterol-lowering effects: Soluble fibers bind to cholesterol molecules in the small intestine and prevent their absorption. The cholesterol-lowering properties of soluble fibers help to prevent coronary heart disease. [13]
  • Postprandial glucose and insulin levels: Soluble fibers slow the absorption and digestion of carbohydrates and lower insulin demand in the body, thereby stabilizing postprandial glucose levels. [14] [15] [16]
  • Improved satiety and weight loss: Soluble fibers make the stomach contents viscous and prolong gastric emptying time. Studies have shown this to help reduce weight, improve BMI, lower body fat, and waist-to-hip ratio. [17]
  • Cancer prevention: Some studies have shown that soluble fibers such as pectin have antioxidant properties and lower the risk of other types of cancer, including breast cancer. [18]
  • Helps to live longer: Fiber intake is associated with decreased prevalence of diabetes type 2, coronary heart disease, stroke, and improved immune function. [19] [17]
  • Nursing, Allied Health, and Interprofessional Team Interventions

In the United States, almost half of all individuals carry a diagnosis of pre-diabetes, diabetes, or metabolic syndrome. [20]  A high fiber diet is associated with physiological health benefits, not just limited to weight loss. An average person in the US consumes about 15 grams of fiber per day, which is only barely half the recommended amount. Diets rich in fiber include the Mediterranean diet, cereals, oat, etc. The Mediterranean diet consists of fruits, vegetables, fish, olive oil, legumes, and beans. The current complex dietary guidelines can be changed to a simpler nutritional approach if dieticians and other health care professionals encourage a shift from the typical Western diet and white flour foods that focus on fiber-rich sources.

Two closely linked mechanisms, epithelial-mesenchymal transition and angiogenesis, contribute to metastasis and recent research has also highlighted the impact of telomere replication on this harmful tumor progression. Standard chemotherapy alone can inadvertently promote drug-resistant CSCs, posing a challenge. Combining chemotherapy with other compounds, including natural ones, shows promise in enhancing effectiveness while minimizing side effects. This study investigated the anti-metastatic potential of Manuka honey, both alone and in combination with 5-Fluorouracil, using a 3D model of colon spheres enriched with CSCs-like cells. In summary, it was observed that the treatment reduced migration ability by increasing the expression of E-cadherin through the downregulation of transcription factors Slug, Snail, and Twist. Additionally, it downregulated pro-angiogenic factors and shortened CSC telomeres by downregulating c-Myc, demonstrating an effective anti-metastatic potential. This study suggests new research opportunities for studying the impact of natural compounds when combined with pharmaceuticals, with the potential to enhance effectiveness and reduce side effects.

sigma 2/2024: Life insurance in the higher interest rate era

Asset-savvy is the new asset-light.

Article information and share options

The surge in interest rates to 15-year highs dramatically improves the outlook for life and annuity insurance. After more than a decade of weak demand driven by low interest rates, profitability is recovering as rises in government bond yields improve investment returns and margins on life insurance products. We expect the operating result for insurers in the largest eight life markets to rise by more than 60% in the five years to 2027, as investment income rises by 40%.

In contrast, the sector missed its cost of capital by nearly 5ppts per year on average in the post-global financial crisis decade. Life insurance stock market indices, a forward-looking proxy for profitability expectations, are now outperforming wider markets as investors recognise the benefit of higher rates. Long-duration business should see the greatest profitability gain in the long term, given compound interest.

Figure 1. Life insurer operating results and investment return, eight key markets, 2021‒27F

Low interest rates made life savings products less attractive. Real premium growth for saving business fell below global economic growth in the decade after the global financial crisis, to just 1.1% annually on average. Today, consumers are moving quickly to buy life products that will secure them higher retirement incomes. We anticipate strong, annuity-driven growth in the life savings market as the interest rate reset makes savings products more attractive. US fixed annuity sales will likely reach a new record again this year, after sales in 2023 were more than twice as high as in any other year before 2022.

Figure 2. US individual annuities direct business, first year and single premiums, 2019-23

The demand boost should help to mobilise the huge private savings needed to narrow the retirement savings gap between current pension assets and the amount populations need to securely fund retirements. We estimate the retirement savings gap for six advanced economies, China and India, at USD 106 trillion in 2022 values.

Figure 3. Retirement savings gap in 2022 values, USD trillion

After only USD 300 billion of premium growth in the entire decade 2010–2019, we predict life insurers will gain USD 1.5 trillion of savings premiums in the 10 years from 2025, to reach USD 4 trillion savings premiums by 2034.

Figure 4. Savings business: forecast additional premiums 2025–34, and comparison with 2010-19, USD billion

Rising interest rates transformed the competitive and operational environment for life insurers from a low growth, low profitability business to one of higher growth and higher returns, especially for asset-intensive business. As a result, the life industry is shifting from returning excess capital to shareholders to a mode of needing sustainable capital growth to support the growth in asset and biometric risks.

Figure 5. Growth in net premiums, net investment income and fee earnings for stock and mutual insurers, indexed, 2012 = 100

In the low-interest rate years from 2009 until rapid monetary tightening in 2022, stock insurers pivoted into capital-light products and used reinsurance transactions to divest their legacy liabilities. Private equity (PE) firms acquired many of these legacy annuity book assets, recognising that low interest rates created an opening for asset managers to generate outperformance from life and annuity portfolios by investing in higher-yielding illiquid and private assets.

Figure 6. European life insurance industry portfolio allocations, 2017-22

PE-owned insurers’ acquisitions gave them stable funding to develop their investment operations and grow their assets under management (AUM) and earnings. For some, insurance assets are now a large share of their total AUM. We estimate that more than USD 1 trillion of life assets have transferred to PE-owned insurers globally since 2009. They own roughly 25% of US individual annuity liabilities and are growing in markets such as Japan.

However, PE-owned insurers face increasing competition from insurer-owned asset managers, as insurers expand their own asset management capabilities, which have benefited from the rise in unit-linked business. We expect further expansion and competition in asset management, with more hybrid product launches.
