Treatment characteristics

Who is this treatment for.

This surgical procedure is suitable for adult transgender women with documented gender dysphoria who have undergone the necessary psychological evaluations and hormonal treatments to reach the final transition stage.

Before the gender reassignment intervention is performed, the following requirements need to be met, as the changes the patient undergoes are irreversible:

  • Being of full legal age.
  •  Have undergone hormonal therapy for a minimum period of 9 months before the gender reassignment intervention.
  • Maintain contact with a psychologist who can rule out the existence of a transitory psychological condition and who certifies that the patient suffers a real process of transsexuality.
  • Have a certificate processed by a psychiatrist that confirms the diagnosis of transsexuality. This is a necessary legal requirement to carry out the surgical procedure.
  • Sign an informed consent that states the irreversibility of the surgery as well as the possible complications resulting from it.

How is the procedure carried out?

Penile Skin Inversion (PSI)  consists of:

  • Making an incision in the midline of the penis.
  • The testicles and spermatic cords are removed.
  • The skin of the penis is used to build the vaginal walls.
  • The vaginal cavity is built between the bladder and the rectum.
  • The urethral canal is redirected and the erectile tissue is removed.
  • The skin of the scrotum is used to create the vaginal lips.
  • Part of the glans (with blood vessels and nerve endings intact) is used to build the clitoris, which will be sensitive and erogenous.

For the more complex vaginoplasty  (Sigma-colon interponate),  which is performed with the intestine, an incision is made in the abdomen through which approximately 15 cm of the large intestine is removed and placed in the perineal area. The rest of the operation, after reconnecting the intestine for proper function, is carried out in the same way as the previous one.

After your time in the recovery room, you will be taken to the ward, where you will remain for 7 to 10 days.

On the ward you will receive the necessary care and rehabilitation to facilitate your discharge and return home.

During your stay in the hospital and for a few weeks after discharge, you will wear a catheter for urination.

What is the result of the treatment?

After Vaginoplasty and recovery, the patient will have a functional, anatomical, aesthetically similar vagina to any woman.

The vagina will allow you to maintain satisfactory sexual activity, with a depth of penetration of at least 10-12 cm, excellent sensitivity and normal urinary function.

The vulva will be covered by the labia minora and majora.

What recovery time do I need in Spain?

If everything goes well, you could return to home after your 15-day check-up, but if possible, it would be recommendable to stay in Spain for three or four weeks in order to monitor your recovery.

You should be fully recovered within two to three months.

Treatment itinerary

Before treatment.

  • Check that you have prepared all the personal documentation (credit card, cash, passport, Identity Card…), medical (reports, prescriptions, images…) and travel (confirmation of accommodation booking, travel tickets…)​.
  • If you take medication of any kind not related to the surgery, do not forget to take with you the necessary amount for your stay in Spain, as well as the medical prescription that will allow you to travel with it without problems.​

During treatment

  • Once you are in Spain, you will have to make an appointment with your specialist. During this visit, the doctor will open your medical record (for which you will be asked to provide all your medical documentation), perform a physical examination and tell you if any further tests are needed to confirm the diagnosis.
  • If it is not necessary and everything is confirmed, your doctor will tell you when you must go to the preoperative study and will confirm the time of admission for the day of your surgery.
  • If the surgeon has told you to stop taking any medication a few days before the surgery, such as anticoagulants, please do not forget to follow the exact instructions given by the doctor or you will not be able to have the operation!.
  • The same goes for any fasting that the doctor indicates, if you do not comply – You will not be able to have the surgery either.

After treatment

  • After your surgery, you must rest and not exert yourself or play sports for at least the first week following surgery. After approximately three or four weeks (depending on the surgery performed), you will have to contact the professional who supervises the hormonation to readjust it.
  • You should follow the instructions given to you by your doctor and the expert physiotherapists to start walking; do not forget that you will need to use crutches so as not to overload the joint and you should avoid any physical overexertion such as from running, carrying weight, sudden rotations or doing impact sports.
  • Remember that your recovery time will vary depending on your age, your physical condition, your lifestyle habits and your attitude.

Spain passes law allowing anyone over 16 to change registered gender

Legislation will also ban conversion therapy, introduce menstrual leave and ease abortion limits

Spain’s parliament has approved new legislation that will allow anyone over 16 to change their legally registered gender, ease abortion limits for those aged 16 and 17, and make the country the first in Europe to introduce paid menstrual leave.

The new transgender law – which was passed despite protests from feminist groups, warnings from opposition parties, and amid tensions between different wings of the Socialist-led coalition government – means that anyone aged over 16 will be able to change their gender on official documents without medical supervision.

However, a judge will need to authorise the change for minors aged between 12 and 14, while those aged between 14 and 16 will need the consent of their parents or guardians. No such changes will be available to those under the age of 12.

The law will also see a ban on conversion therapy – punishable by hefty fines – and an end to public subsidies for groups that “incite or promote LGBTIphobia”.

The new abortion legislation does away with a a 2015 measure, introduced by the conservative People’s party (PP), which requires women aged 16 and 17 to obtain parental consent for abortions. It also scraps the current three-day period of reflection for those seeking a termination, and aims to make it far easier for women to access abortion in public hospitals and clinics.

Thursday’s vote introduces up to five days of menstrual leave for women who have incapacitating periods. According to the Spanish Gynaecological and Obstetric Society, a third of women experience dysmenorrhoea, or painful menstruation. Accompanying measures include the free provision of free sanitary products in schools, prisons and women’s centres to tackle “period poverty”.

Spain ’s equality minister, Irene Montero, began her speech to congress on Thursday by thanking Spain’s LGBTQ+ community and trans collectives for helping to get the law passed – and for “saving many lives” in the absence of government intervention.

Montero, who belongs to the Socialists’ junior coalition partners, the far-left, anti-austerity Podemos partner, said the new law was about the state guaranteeing basic rights.

“This is a law that recognises trans people’s right to freely decide their gender identity, she said. “It stops trans realities being treated as abnormalities. Trans people aren’t sick people; they’re people – full stop. They are who they are – full stop. Trans women are women – full stop. From today, the state recognises that.”

However, the PP accused the government of introducing more ill-considered legislation in the wake of the botched “only-yes-means-yes” sexual consent law that has allowed some convicted sex offenders to have their sentences retrospectively reduced. The Socialists have promised to correct the “undesired effects” of that law, angering their partners in Podemos, which pushed the new consent legislation and still defends it.

Before the trans law passed by 191 votes in favour, 60 against and with 91 abstentions, the PP warned the government it was going too far, too fast.

“We all know of other countries that have backtracked on their ‘trans laws’ because they now know that they got ahead of themselves and that that caused a lot of suffering,” said a party spokesperson, María Jesús Moro. “Let’s not have the same thing here.”

She added: “We don’t want to see a new and unbearable rollcall of victims just days after this new law comes into effect. We don’t want a remake of the ‘only-yes-means-yes’ law.”

However, the new law was welcomed by Uge Sangil, the head of FELGBTI+, Spain’s largest LGBTQ+ organisation.

“We’re celebrating the fact this law has passed after eight years of tireless work to obtain rights for the trans community,” Sangil told Agence France-Presse outside parliament.

“We’re winning human rights with the free determination of gender … From today, our lives will change because we are not ill.”

In recent years, several European nations that pioneered transgender legislation have had second thoughts.

Among those to have reimposed restrictions are Sweden and Finland, while in the UK, Westminster last month blocked a Scottish trans rights law similar to Spain’s.

The bitter dispute over transgender issues played a role in Wednesday’s shock resignation of Scotland’s first minister, Nicola Sturgeon.

Although she had championed the law, Sturgeon became entangled in a major row over transgender women entering all-female prisons, after a rape case that caused a public outcry.

A year ago, Sweden decided to halt hormone therapy for minors except in very rare cases.

In December, it limited mastectomies for girls wanting to transition, to a research setting, citing the need for “caution”.

The decision followed moves by Finland, which decided to restrict gender reassignment hormone treatment for similar reasons in 2020.

The Spanish government on Thursday approved a law allowing people 16 and older to change their legally registered gender without undergoing psychological and medical evaluations to show gender dysphoria, becoming one of the few countries to allow such gender change by self-declaration.

Spain’s minister of equality, Irene Montero, said on the Parliament floor on Thursday that the new law recognized transgender people’s right to free determination and prevented being transgender from being treated as a pathology.

“Trans people are not ill people,” she said. “They are people, full stop.”

Spain is among the first countries to pass such a law, following countries like Denmark and Argentina . Similar proposals have divided public opinion elsewhere. Last month, Britain’s government overruled Scotland’s Parliament for the first time, blocking legislation that would have allowed transgender people to have the gender with which they identify legally recognized by making a declaration. The plan would have removed a requirement for “evidence of a diagnosis of gender dysphoria.”

Spanish lawmakers on Thursday also gave final approval to a law that gives women paid time off if they are diagnosed with severe menstrual pain , becoming the first country in Europe to do so. They also extended access to abortion to minors 16 and older without the consent of a parent or guardian.

The gender law has created friction between the right and the left but also within the Socialist Party, Spain’s biggest liberal party.

Víctor Gutiérrez, the L.G.B.T. secretary of the Socialist Party, praised it, writing on Twitter that it was a “law that improves the life of millions of people.”

But Carmen Calvo, a prominent Socialist politician and the former deputy prime minister, abstained from voting on the law, and a Socialist senator said on Twitter that lawmakers should reject it in the name of feminism and socialism.

“Laws that are being called off in other countries must not be imposed by the will of a minority,” Feministas Socialistas, an association of socialist feminists, said in a statement.

But for transgender rights activists, it was a belated victory.

“The Calvary is over,” Mar Cambrollé Jurado, a transgender rights activist, wrote on Twitter. “Today is a historic day for trans people.”

Under the new law, children between 14 and 16 will be able to legally change their gender in the civil registry if they are accompanied by their parents or a legal guardian, and those between 12 and 14 will need a judge’s authorization.

After the initial request for a legal gender change, the applicant will need to ratify their decision within three months.

The law also banned conversion therapies intended to change a person’s sexual orientation or gender identity.

Gender confirmation surgery (GCS), known clinically as genitoplasty, are procedures that surgically confirm a person's gender by altering the genitalia and other physical features to align with their desired physical characteristics. Gender confirmation surgeries are also called gender affirmation procedures. These are both respectful terms.

Gender dysphoria , an experience of misalignment between gender and sex, is becoming more widely diagnosed.  People diagnosed with gender dysphoria are often referred to as "transgender," though one does not necessarily need to experience gender dysphoria to be a member of the transgender community. It is important to note there is controversy around the gender dysphoria diagnosis. Many disapprove of it, noting that the diagnosis suggests that being transgender is an illness.

Transfeminine Transition

Transfeminine is a term inclusive of trans women and non-binary trans people assigned male at birth.

Gender confirmation procedures that a transfeminine person may undergo include:

  • Penectomy is the surgical removal of external male genitalia.
  • Orchiectomy is the surgical removal of the testes.
  • Vaginoplasty is the surgical creation of a vagina.
  • Feminizing genitoplasty creates internal female genitalia.
  • Breast implants create breasts.
  • Gluteoplasty increases buttock volume.
  • Chondrolaryngoplasty is a procedure on the throat that can minimize the appearance of Adam's apple .

Feminizing hormones are commonly used for at least 12 months prior to breast augmentation to maximize breast growth and achieve a better surgical outcome. They are also often used for approximately 12 months prior to feminizing genital surgeries.

Facial feminization surgery (FFS) is often done to soften the lines of the face. FFS can include softening the brow line, rhinoplasty (nose job), smoothing the jaw and forehead, and altering the cheekbones. Each person is unique and the procedures that are done are based on the individual's need and budget,

Transmasculine is a term inclusive of trans men and non-binary trans people assigned female at birth.

Gender confirmation procedures that a transmasculine person may undergo include:

  • Masculinizing genitoplasty is the surgical creation of external genitalia. This procedure uses the tissue of the labia to create a penis.
  • Phalloplasty is the surgical construction of a penis using a skin graft from the forearm, thigh, or upper back.
  • Metoidioplasty is the creation of a penis from the hormonally enlarged clitoris.
  • Scrotoplasty is the creation of a scrotum.

Procedures that change the genitalia are performed with other procedures, which may be extensive.

The change to a masculine appearance may also include hormone therapy with testosterone, a mastectomy (surgical removal of the breasts), hysterectomy (surgical removal of the uterus), and perhaps additional cosmetic procedures intended to masculinize the appearance.

Paying For Gender Confirmation Surgery

Medicare and some health insurance providers in the United States may cover a portion of the cost of gender confirmation surgery.

It is unlawful to discriminate or withhold healthcare based on sex or gender. However, many plans do have exclusions.

For most transgender individuals, the burden of financing the procedure(s) is the main difficulty in obtaining treatment. The cost of transitioning can often exceed $100,000 in the United States, depending upon the procedures needed.

A typical genitoplasty alone averages about $18,000. Rhinoplasty, or a nose job, averaged $5,409 in 2019.  

Traveling Abroad for GCS

Some patients seek gender confirmation surgery overseas, as the procedures can be less expensive in some other countries. It is important to remember that traveling to a foreign country for surgery, also known as surgery tourism, can be very risky.

Regardless of where the surgery will be performed, it is essential that your surgeon is skilled in the procedure being performed and that your surgery will be performed in a reputable facility that offers high-quality care.

When choosing a surgeon , it is important to do your research, whether the surgery is performed in the U.S. or elsewhere. Talk to people who have already had the procedure and ask about their experience and their surgeon.

Before and after photos don't tell the whole story, and can easily be altered, so consider asking for a patient reference with whom you can speak.

It is important to remember that surgeons have specialties and to stick with your surgeon's specialty. For example, you may choose to have one surgeon perform a genitoplasty, but another to perform facial surgeries. This may result in more expenses, but it can result in a better outcome.

Gender confirmation surgery is very complex, and the procedures that one person needs to achieve their desired result can be very different from what another person wants.

Each individual's goals for their appearance will be different. For example, one individual may feel strongly that breast implants are essential to having a desirable and feminine appearance, while a different person may not feel that breast size is a concern. A personalized approach is essential to satisfaction because personal appearance is so highly individualized.

Gender-affirming care encompasses a broad range of psychological, behavioral and medical treatments for transgender, nonbinary and gender-nonconforming people.

The care is designed to “support and affirm an individual’s gender identity” when it is at odds with the sex they were assigned at birth, as defined by the World Health Organization.

What is gender-affirming surgery?

Gender-affirming surgery refers to the surgical and cosmetic procedures that give transgender and nonbinary people “the physical appearance and functional abilities of the gender they know themselves to be,” according to the American Society of Plastic Surgeons. It is sometimes called gender reassignment surgery.

There are three main types of gender-affirming surgeries, per the Cleveland Clinic:

Top surgery , in which a surgeon either removes a person’s breast tissue for a more traditionally masculine appearance or shapes a person’s breast tissue for a more traditionally feminine appearance. 

Bottom surgery , or the reconstruction of the genitals to better align with a person’s gender identity.

Facial feminization or masculinization surgery , in which the bones and soft tissue of a person’s face are transformed for either a more traditionally masculine or feminine appearance.   

Some people who undergo gender-affirming surgeries also use specific hormone therapies. A trans woman or nonbinary person on feminizing hormone therapy, for example, takes estrogen that’s paired with a substance that blocks testosterone. And a trans man or nonbinary person on masculinizing hormone therapy takes testosterone.

Gender-affirming surgeries and treatments are the recommended course of treatment for gender dysphoria by the American Medical Association. Gender dysphoria is defined as “clinically significant distress or impairment related to gender incongruence, which may include desire to change primary and/or secondary sex characteristics,” according to the American Psychiatric Association.

Some LGBTQ+ advocates and medical professionals feel that gender dysphoria shouldn't be treated as a mental disorder, and worry that gender dysphoria’s inclusion in the DSM-5 — the authoritative source on recognized mental health disorders for the psychiatric industry — stigmatizes trans and nonbinary people.

How much does gender-affirming surgery cost?

Gender-affirming surgery can cost between $6,900 and $63,400 depending on the precise procedure, according to a 2022 study published in The Journal of Law, Medicine and Ethics.

Out-of-pocket costs can vary dramatically, though, depending on whether you have insurance and whether your insurance company covers gender-affirming surgeries.

There are also costs associated with the surgery that may not be represented in these estimates. Additional costs may include:

The cost of psychiatric care or therapy, as most insurance companies and surgeons require at least one referral letter prior to the surgery. An hour of therapy can cost between $65 and $250, according to Good Therapy, an online platform for therapists and counselors. 

Time off work. After bottom surgery, you can expect to miss six weeks of work while recovering. Most people miss around two weeks of work after top surgery. 

Miscellaneous goods that’ll help you recover. For example, after bottom surgery, you might need to invest in a shower stool, waterproof bed sheets, cheap underwear and sanitary towels. Top surgery patients may need, depending on the procedure, a mastectomy pillow, chest binder and baggy clothes.

Is gender-affirming surgery covered by insurance?

It’s illegal for any federally funded health insurance program to deny coverage on the basis of gender identity, sexual orientation or sexual characteristics, per Section 1557, a section of the Affordable Care Act. Section 1557 doesn’t apply to private insurance companies, though, and several U.S. states have passed laws banning gender-affirming care.

The following states have banned gender-affirming surgery for people under 18 years old, according to the Human Rights Campaign: Alabama, Arkansas, Florida, Georgia, Idaho, Indiana, Iowa, Kentucky, Louisiana, Mississippi, Missouri, Montana, Nebraska, North Carolina, North Dakota, Oklahoma, South Dakota, Tennessee, Texas, Utah, West Virginia. In four of these states — Alabama, Arkansas, Florida and Indiana — court injunctions are currently ensuring access to care.

And these states have either passed laws — or have governors who issued executive orders — protecting access to gender-affirming surgery, according to the Movement Advancement Project, a public policy nonprofit: California, Colorado, Connecticut, Illinois, Maryland, Massachusetts, Minnesota, New Jersey, New Mexico, New York, Oregon, Vermont and Washington, D.C.

But even if your state has enshrined protections for gender-affirming care, some private insurance companies may consider surgeries “cosmetic” and therefore “not medically necessary,” according to the Transgender Legal Defense and Education Fund. If you have private insurance or are insured through your employer, contact your insurance company and see if they cover gender-affirming care. Also, ask about any documentation the insurance company requires for coverage.

The Williams Institute estimates that 14% of trans Americans currently enrolled in Medicaid live in states where such coverage is banned, while another 27% of trans Americans live in states where coverage is “uncertain,” because their state laws are “silent or unclear on coverage for gender-affirming care.”

Because of Section 1557, Medicaid is federally banned from denying coverage on the basis of sex or gender; among the roughly 1.3 million transgender Americans, around 276,000 have Medicaid coverage, according to a 2022 report from the Williams Institute.

If your private insurance company won’t cover gender-affirming care, and you’re unable to obtain coverage through the federal marketplace, consider these sources:
